REQUIRED WAIVERS- IMPORTANT Due to the COVID-19 pandemic, there are additional waivers that are required prior to the league start. The forms are here as required: Declaration of Compliance – COVID-19 – To be completed by all members Release of Liability, waiver of claims and indemnity agreement – To be completed by all members 18 years of age and older Informed consent and assumption of risk agreement – To be completed for participants 16 and 17 years of age Please review, sign, and submit the completed waivers to kwsc.communications@gmail.com with your full name and team name in the subject line by October 8th, 2021.
